It is very important that you repeat your subscription box a few times as this will maximize your response. Also if you include a graphic ecover to symbolize your free report this will significantly boost your conversion rate.

Finally, you need to have a thank you page as well. I suggest that you include a download link in your first autoresponder email. If you put it on the thank you page it will encourage people not to confirm their email address to receive the free report.
